Assuming that a patient gave you access to all the items listed in the question above, would it also be essential to have access to their previous physician and hospital medical records in order for you to evaluate a patient thoroughly? Please pick the best answer.
#Response DateOther (please specify)
.11/14/2007 7:48:00 PMThe right to access to the medical file is granted for the patient. It must absolutely be the same in the other way, from patient to his/her MD
.11/14/2007 8:49:00 PMagain getting succinct relevant info is needed and we already see repeated reguritations of prior info that just imede focusing on what is needed to help the patient now.
.11/22/2007 8:40:00 PMRecal goal: Patient Focused, Community Based, Longitudinal Health Record. A cloud of data that follows the patient wherever he/she needs. It should be accurate, complete, current, consistent.
.11/23/2007 1:20:00 AMyes, number 3
.11/23/2007 3:29:00 AMI t may be hard to evaulate information that the patient has stored. It may be incomplete or out of context.
.11/23/2007 3:40:00 AMOf course, not necessary in very simple situations (e.g. suturing cut finger), but very important in most medical & significant traumatic situations.
.11/23/2007 8:22:00 AMThere has to be be a search engine of a "Semantic Web" dimension to parse the data created by a life time of medical data
.11/23/2007 2:11:00 PM"also be HELPFUL to have access... " "Having access to other healthcare provider medical records is very important in SOME situations, especially in patients with complex medical issues."
.11/23/2007 3:57:00 PMBut when I need them, I really need them and cannot function appropriately without them.
.11/23/2007 6:00:00 PMI need old records in some situations
.11/23/2007 10:59:00 PMI care for patients with multi-system disorders, thus maximal communication between providers is essential.
.11/24/2007 12:15:00 AMBut, the records need to be WELL ORGANIZED!!!! (We can't hunt through pages of scanned documents for the last echo...
.11/24/2007 12:39:00 AMThe less accurate & medically sophisticated the patient is (or the more drug-seeking they are) the more important it is.We should also be able to access limited police records (to see if a crime has been reported) and pharmacy records (to see if a prescription has been filled, or multiple prescriptions from multiple physicians) for those patients who appear on holidays, nights, & weekends (usually from out of state) claiming that their pain medicine or prescriptions were stolen, lost, or eaten by their pet.
.11/24/2007 12:36:00 PMi rarely need the old records
.11/25/2007 11:12:00 PMThis will cause more confusion and even a bigger burden on already overburdened physicians. The problem is not lack of information but the limited time for physicians to be able to assimilate it.
.11/28/2007 9:02:00 PMThis is much more important in patients with complex medical issues. In pts without compex histories, old records may not add important info, but you never know.
